Please use this form to communicate your suggestions/ideas/concerns with the Equity Committee. Example usages include: 

• Concerns you would like the committee (or select members of the committee) to address;
• Incidents you would like to report (though see below for more appropriate resources to report incidents of harassment);
• Suggestions for changes you would like to see in the department;
• Ideas you would like the committee to consider the committee implementing;
